What is the difference between a private and public promo?

Public promos have a public URL and anyway stumbling upon that URL will be able to play your promo.

Promos that are not public are only accessible via a url that is unique to every contact who has access to that promo. When a promo is private, we are able to track granual user activity such as track plays and downloads.
